Title : The vocal fold AP shortening and advancement of ant. commissure for feminization of the voice
Objectives : To evaluated results of the modification of vocal fold shortening surgery in patients with androphonia and male-to-female transsexual patients.
Method : Retrospective study of thirty-three patients who underwent the vocal fold shortening between 2004 and 2006 have been done. The modified vocal fold shortening has been performed to patients with androphonia(n=8), androgenital syndrome(n=2), aplastic anemia treated with androgen(n=3) and to male-to-female transexual patients(n=20). The subjective and perceptual assessment, aerodynamic and acoustic assessment, and videostroboscopic assessment were evaluated before and after phonoplasty. All patient was performed voice rehabilitation program after postoperative 2 months.
Results : The average preoperative F0 was 137.3Hz and the mean postoperative F0 achieved was 211.5Hz. Duration of follow up ranged from 6 to 28 months. The average increase in F0 was 74.2 Hz after phonoplasty and voice rehabilitation program. In subjective asssessment, voice feminity was increased. Acoustic assessment presented sPPQ, sAPQ, NHR showed within normal range and subglottic pressure and regularity of mucosal wave of vocal fold maintained in normal range. These fiindings suggest that patients have unartificial voice and could make soft phonation after surgery.
Conclusion : The vocal fold AP shortening and advancement of anterior commissure could be considered an effective method to perform for voice feminization with unartifial voice.
